If you are making a gift to Crossroads, be sure to request a Crossroads Remembrance Card.

Simply contact us and request the number of cards you want to give. We will send the cards and mailing envelopes to you. Each card contains a message explaining to the recipient that a donation has been made in their name to further the work of Crossroads. There is a space for you to write in the name of honoree, as well as an "At the request of" line for your name. After you fill in this information, the card is ready to present.

A donation form is included for each card and your donation to Crossroads may be in the amount of your choosing and need not be sent to us until you use the Remembrance Card.

Crossroads Remembrance Cards are a wonderful way to honor anyone special in your life or to send in memory of someone who has passed away. They also make lovely Christmas, birthday, or anniversary gifts so keep several on hand as a great way to mark any occasion.
